We are looking for a Case Manager to ensure the people supported are working toward their chosen goals.
The successful candidate will assist in identifying community resources, provide support and intervention where needed in the workplace and act as a liaison with family members.
In addition, they will serve as an advocate for the people supported on his/her caseload in ensuring all necessary services are provided.
Responsibilities Ensures that all Person-Centered Plans are completed according to The Arc NCR policy and that they are fully implemented by staff Provides advocacy for people supported served based on their personal choice and informed decision making regarding their desires.
Facilitates team meetings and family conferences to problem solve and develop plans of action as needed.
Prepare and maintain records and case files within iCare Manager, including documentation such as people supported personal and eligibility information, services provided, narratives of people supported contacts, and relevant correspondence.
Develop PCP plans that fit people supported aptitudes, education levels, physical abilities, and career goals.
Monitor LTSS for all PCP funding.
Monitor and record people supported progress to ensure that goals and objectives are met.
Confer with people supported to discuss their options and goals so that organizational programs and plans for accessing needed services can be developed.
Maintain close contact with people supported during job training and placements to resolve problems and evaluate placement adequacy.
Maintain close contact with people supported, families, providers as people move into Community Living program to ensure all information is collected for a smooth transition.
Confer with physicians, psychologists, occupational therapists, and other professionals to develop and implement client rehabilitation programs.
Arrange for physical, mental, academic, Employment, and other evaluations to obtain information for assessing people supported needs and developing PCP plans.
Analyze information from interviews, educational and medical records, consultation with other professionals, and diagnostic evaluations to assess people supported abilities, needs, and eligibility for services.
Identify barriers, such as inaccessible work sites, inflexible schedules, and transportation, and work with people supported to develop strategies for overcoming these barriers.
Collaborate with people supported families to implement PCP plans such as behavioral, residential, social, and employment goals.
Participate in job development and placement programs, contacting prospective employers, placing people supported in jobs, and evaluating the success of placements.
Arrange for on-site job coaching or assistive devices, such as specially equipped wheelchairs, to help people supported adapt to work or school environments.
Develop and maintain relationships with community referral sources, such as schools and community groups.
Works in conjunction with other organizational departments to ensure the needs of people supported served are met utilizing all resources available.
Assist in the training and onboarding of new team members as necessary.
Completes trainings on PCPs for annual plans and auditing tracking monthly.
Completes, maintains, and submits required reports and documentation in a timely manner.
Identifies informal supports available in the community for people supported served Keeps current and accurate people supported records, case files, record notes and medical records.
Ensure compliance with all regulatory and accrediting bodies.
Takes responsibility for safety hazards in all areas including, but not limited to, administrative buildings, and vehicles.
Facilitates team meetings and family conferences to problem solve and develop plans of action as needed.
Completes, maintains, and submits required reports and documentation in a timely manner.
Schedules and coordinates the attendance of all necessary parties for all meetings for people supported including but not be limited to annual people supported planning meetings, emergency meetings, and interim meetings for people supported receiving services from the Agency.
Education, Training, Experience, and requirements needed:
High school diploma required; Associate or Bachelor's Degree in Social Work or Human Services preferred At least one year of excellent experience managing a caseload of 20 or more; two (2) years preferred Fluent in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, etc.
Access to reliable transportation Active valid driver's license and driving record that has no more than two (2) points.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
